Alongside breast augmentation with implants, breast reduction is one of the most commonly performed procedures in aesthetic surgery. On average, German women have a cup size of 75B. However, if the bust size is significantly larger than average, this can be very stressful both physically and psychologically. For women with a large bust size, sagging breasts can be a problem. They represent an additional, heavy burden. Oversized breasts can often lead to back pain, neck and shoulder tension, as well as therapy-resistant inflammation and eczema in the skin-overlapping breast fold. For this reason, many women opt for surgical breast reduction.
The surgical steps involved in breast reduction are comparable to those involved in a breast lift. At his specialist clinic for aesthetic plastic surgery, Dr. Münzberg also removes a corresponding amount of glandular tissue and excess skin during breast reduction. The surgical technique known as the “internal bra,” as used in breast lifts, is also possible in breast reduction. In addition, the breast can be reshaped during the procedure to create a fuller cleavage.
A detailed consultation is essential before any surgical procedure. During your appointment at the Clinic for Aesthetic Plastic Surgery with specialist Dr. Münzberg, you can ask any questions you may have about the operation and express your wishes and expectations. Once all questions have been answered, a physical examination will be carried out and blood samples will be taken. We recommend that you have a mammogram before the procedure.
Please remember that it is important not to take any blood-thinning medication for two weeks before the procedure. You should also refrain from nicotine and alcohol for several weeks. Please inform Dr. Münzberg about your current medication before the treatment.
Experienced plastic surgeons such as Dr. Münzberg now prefer to use the scar-saving LeJour technique to minimize scarring. In the past, the so-called T-incision was used, which left a scar that extended beyond the crease under the breast. Dr. Münzberg also performs breast reductions using the scar-saving LeJour technique. The surgeon only uses an alternative technique if the circumstances, such as very large and sagging breasts, require it. In this case, a T-incision is made and an additional scar runs along the breast fold. A barely visible scar runs around the areola, and a vertical scar starts at the areola and runs down towards the breast fold. This new breast reduction technique eliminates the scar that used to be created in the breast fold area by the so-called T-incision. Please note that the final result of the surgery will only be visible after 3 to 6 months.
Thanks to the new technique of the so-called internal bra in breast reduction, the treatment results remain intact for many years, despite the aging process and connective tissue weakness. With this innovative method, the experienced plastic surgeon Dr. Münzberg prepares two strips approximately 1 cm wide from the excess skin of the breast. He then places these strips crosswise, extending from the nipple to the breast fold. If you have any further questions, please do not hesitate to contact us. The surgeon attaches the strips to the chest muscle with sutures to prevent the remaining glandular tissue from sagging again.
In some cases, it may be advisable to perform liposuction under the armpits in addition to the breast lift in order to achieve a harmonious result, as the fatty tissue of the breast can extend into the subaxillary area. This can create a harmonious appearance (silhouette) after breast reduction. In this case, liposuction can be performed at the same time as breast reduction.
Breast reduction performed in this way preserves the sensitivity of the nipples. This means that it is still possible to breastfeed children later on, as the milk ducts are located outside the area of the operation.
If the breasts are very large, specialist Dr. Münzberg is unfortunately unable to use the scar-saving breast reduction technique. In this case, the classic T-incision is used. With this type of breast reduction, an additional scar is required in the area of the breast fold, and the nipple must be moved as a free skin graft.
However, it should be noted that the nipple loses all functions, including sensitivity and the ability to breastfeed.
Breast reduction surgery can be performed on an outpatient or inpatient basis under general anesthesia and takes approximately 3–4 hours.
After your surgery, you will be admitted to our clinic for one night to ensure optimal care. Our nursing staff will take care of you and make sure that you are comfortable. The pain after breast reduction surgery is usually minimal and can be easily controlled with mild painkillers. The pain subsides after about 3 to 5 weeks.
If you wish, the operation can also be performed on an outpatient basis.
As an experienced aesthetic surgeon, Dr. Münzberg does not use self-dissolving sutures for better wound healing, as these can affect the final result. The sutures must be removed approximately 2 weeks after the breast surgery. The scars should then be treated with a silicone gel or plaster for at least 8 weeks so that they heal as finely and inconspicuously as possible.
Self-dissolving sutures contain a high sugar component, which allows them to be dissolved by the body.
In order not to put additional strain on the healing process, we recommend that you wear a support bra/special bra day and night for the first 3 months. This relieves pressure on the breast, reduces swelling, and promotes wound healing. We also recommend that you refrain from sunbathing and saunas for the first 6–12 weeks. Sports activities should also only be resumed after the recovery period. When exercising, it is important to wear a snug-fitting and comfortable sports bra.
Breast reduction may be necessary for medical reasons and can lead to physical discomfort. In this case, health insurance will cover the costs of the operation after an expert opinion has been submitted and the medical service has confirmed the medical necessity. Breast reductions and lifts are performed at the clinic of specialist Dr. Münzberg.

The desire for breast reduction surgery is usually medically justified. Particularly large breasts can lead to compensatory postures, back problems, or muscle tension. Many patients also report various types of skin inflammation of different causes in the area of the inframammary fold. Excessively large breasts can also be a hindrance during physical activity. However, some women simply wish to undergo breast reduction surgery for purely aesthetic reasons.
A detailed consultation takes place prior to breast reduction surgery. During this consultation, plastic surgeon Dr. Münzberg explains the surgical techniques and the steps involved in the procedure to the patient. In particular, he also discusses potential risks. Preparations for the surgery include a blood test, an EKG, a pre-anesthesia consultation, and the signing of a written consent form.
There are two proven techniques for breast reduction:
The traditional method involved the so-called T-incision, which left a scar extending beyond the inframammary fold.
A newer and now preferred surgical technique is the scar-minimizing Lejour technique
At Dr. Münzberg’s specialist clinic, breast reduction is performed using the scar-minimizing Lejour technique. This involves a barely visible scar that runs around the areola and vertically downward toward the inframammary fold. The additional scar that used to result from the so-called T-incision is eliminated. It is important to note that with this surgical technique, the final result is not visible until 3 to 6 months after the procedure. Postoperatively, the patient should wear a support and compression bra for at least 3 months.
Using a new technique, Dr. Münzberg also creates a new internal bra, ensuring that the result lasts for many years despite the aging process and weakening of connective tissue. With this new technique, he cuts two strips approximately 1 cm wide from the dermis (from the excess skin of the breast), which he arranges in a cross pattern. These extend from the nipple to the inframammary fold. Finally, Dr. Münzberg secures these to the pectoral muscle.
In addition, liposuction under the armpits is advisable and beneficial in some cases. The fatty tissue of the breast extends into the subaxillary area, so that after breast reduction, the silhouette is not harmonious. In this case, liposuction can be performed simultaneously with the breast reduction.
With this surgical technique, the sensitivity of the nipples is preserved, so that breastfeeding is possible if the patient later wishes to have children, since the milk ducts are not located in the surgical area.
In the case of very large breasts, Dr. Münzberg can no longer use the scar-minimizing technique. In such cases, the T-incision must be used. With this approach, a scar in the area of the inframammary fold cannot be avoided, and the plastic surgeon must relocate the nipple as a free skin graft. As a result, the nipple loses all its functions, including sensitivity and the ability to breastfeed.
The surgical procedure can be performed on an outpatient basis or as an inpatient procedure under general anesthesia.
After a breast reduction, the patient is given two elastic straps (known as “Stuttgart straps”) with Velcro fasteners, which she must wear for 5 days. She should then wear a support and compression bra for 12 weeks. Dr. Münzberg removes the sutures around the nipple after 10 days and the vertical sutures after 16 days. Showering is permitted again after that. The patient should not resume exercise until approximately 6 weeks later.
The follow-up appointments at the specialist clinic after breast reduction surgery are:
- the 1st post-op day for dressing changes and removal of the drains
- the 5th day for dressing changes
- the 10th day for partial removal of the sutures
- the 16th day for removal of the remaining sutures
- after 3 weeks
- after 6 weeks
- after 3 months
- after 6 months
- And finally, after one year.
Like any surgery, breast reduction surgery can involve risks. Possible complications include: impaired wound healing, bruising, prolonged swelling, pain, redness, and discoloration from hematomas, as well as wide, raised, and visible scarring. Loss of sensation and/or circulatory problems in the nipples, loss of fat or glandular tissue with residual asymmetry and/or dimpling in the breast area, so-called “dog-ear” formation, skin pouches, or excess skin, which should be corrected on an outpatient basis under local anesthesia after six months.

With the amendments to the German Law on the Advertising of Medicinal Products that took effect in October 2012, the ban on comparative “before-and-after” images was somewhat “relaxed” (→ see the ruling of the Higher Regional Court of Celle; Case No. 13 U 160/12). The ban on before-and-after images now applies only to visual depictions that are abusive, repulsive, or misleading regarding changes to the human body resulting from diseases or injuries. Due to this rather broadly interpretable wording (when is a visual representation abusive, repulsive, or misleading?), specialist Dr. Münzberg continues to refrain from using before-and-after images on his website. Dr. Münzberg’s clinic also complies with the German Law on the Advertising of Medicinal Products when it comes to stating the prices of surgical procedures, as this law prohibits misleading advertising. Under the law, it could appear as though the outcome of a surgical procedure is for sale and that success is guaranteed. For this reason, this website does not list the costs of breast reduction surgery.
